    Download this FREE Father’s Day Printable questionnaire for your children to fill out and give to dad on Father’s Day! Two versions available; one with a place to insert a picture and another without a photo!

    If you are looking for something fun, homemade, and inexpensive to give to Dad this Father’s Day I think you’ll love this Father’s Day questionnaire printable I created. I couldn’t find much out on the web and wanted to have something to give to my husband like I did for myself for Mother’s Day!

    Jump to the bottom to download and print the Questionnaire with Photo or the Questionnaire without photo. ((Download will only work on a desktop.))

    Time needed: 30 minutes.

    Father’s Day Questionnaire

    1. Fill Out

      Have your child (or yourself if child is to young) fill out the questionnaire.

    2. Print Photo

      Print a photo if desired. Attach photo to the printed filled out worksheet using tape, glue, or cut slits as pictured above.

    Not only will dad love this–it’s great writing practice for the kids that can write. After having a few weeks off from school my Kindergartner (now 1st grader) hadn’t written in a while and it was a struggle. Reminding me that we need to add more writing to our daily routine!

    FAQs

    How do you make slits in paper for photos?

    To attach the photo without an adhesive you can make parallel slits about ¼ inch from the colored boxes of the design. Using an exacto knife on a cutting mat, place the photo near the area you want to cut. Cut a small 1-inch slit (depending on the size) of the photo. If you don’t have an exacto knife you can use a very sharp pair of scissors (using one end to make the slits).
